Imagine having lived your entire life in a hospital and not knowing how the wind feels on your face. Imagine turning sixteen and not knowing if you will have a family to call your own. For the medically fragile children who enter our Special Medical Foster Care & Adoption Program, NAC provides real help and real hope for a nurturing and permanent home and a bright future.

Special Medical Foster Care & Adoption Program

When a medically fragile child is placed in foster care at NAC, a multidisciplinary team of social workers, behavior specialists, medical, mental health, educational, and recreational staff work simultaneously with the birth and foster families toward reunification, and if that is not possible, toward adoption by a loving family. As a result, NAC's children achieve permanency in nurturing and supported homes more quickly than citywide averages.

Therapeutic Foster Boarding Home Program

NAC was the first child welfare agency in New York State to serve children in family-based care who have dual diagnoses – special medical needs in combination with serious behavioral/mental health needs.

Homefinding

NAC is successful because of the quality and commitment of our foster parents. NAC recruits, trains and partners with caring and capable foster families throughout the metropolitan area. NAC's children are not moved from home to home and foster parents frequently become adoptive parents.

Post Legal Adoption Network (PLAN)

Adoption is a lifelong process and adoptive families of medically fragile children, in particular, need support. As a nationally recognized program, NAC's Post Legal Adoption Network is designed to train, assist and strengthen the capacity of families who have adopted one or more children with special medical needs.
