As a Care Manager with NAC, you’ll be a pivotal part of our Champions for Children’s Health Department—a NYS initiative dedicated to enhancing the care of children up to age 21 with intricate medical and behavioral conditions. This role is ideal for someone passionate about coordinating care, advocating for children, and working collaboratively with a diverse team to improve outcomes and streamline services.
