New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan dedicated to serving children with special healthcare needs and their families. To assist in achieving this mission, NAC is currently seeking a PPSP Social Worker/Case Planner to join its Adoption and Kinship Support.

The NAC Family Permanency Center offers families formed through adoption or guardianship, a place where they can find the support, advocacy, information, and a range of professional health and social services needed to help them create or maintain a successful and permanent home.

• Position Title: Post-Permanency Support Program Social Worker/ Case Planner
• Position Type: Full Time (Hybrid)
• Salary Range: Master’s level- $60,000- $65,000 / Bachelor’s level-$50,000-$55,000
• Location: 825 7th Avenue NY NY, 10019
• Department: Adoption & Kindship Support Post-Permanency Support Program

Description of Responsibilities:

• Provides supportive services to adoptive and kinship families post-finalization with the goal of strengthening and maintaining placements and preventing disruptions/dissolutions
• Manage a caseload of pre and post adoptive children and families. Provide individual and family counseling, including crisis counseling around adoption permanency and adoption related issues. Assist families in navigating open adoptions. Provide advocacy, information and referrals to other NAC departments and outside agencies as requested.
• Conduct office, home and community visits as required by each case.
• Responsible for documentation of services provided, including progress notes for monthly contacts, referrals, statistics and psychosocial summaries, etc.
• Screen new referrals for pre and post adoption services. Conduct intake interviews for new families.
• Formulates and implements individualized service plans to identify family goals and needs.
• Participated as a treatment team member at Foster Care & Adoption Department Meetings, Rounds in the absence of Supervisor, Family Team Conferences, Case Reviews and Adoption Disclosure Conferences.
• Participate in individual and group supervision. Participate in agency staff meetings and clinical in-service training.
• Attend outside adoption meetings and/or adoption fairs at the request of the Supervisor.
• Participates in preparing foster families for adoption. Develop and facilitate other group support for children and parents.
• Participates in program development and evaluation; provides consultation to foster care and adoption social workers.
• Compliance with all NAC policies and procedures, foundation and grant requirements and NYS laws.

Required Education and Experience:

• Masters in social work Preferred
• Bachelor’s level degree in social work, psychology, or human service
• Bachelor’s level degree in any field with at least two years of case management experience
• General knowledge of adoptive and kinship families
