New Alternatives For Children, Inc. (NAC) is a private, non-profit health care and social services agency that provides services to children with special medical, developmental and mental health needs and their families. We are currently seeking an Program Assistant/Intake Coordinator to provide administrative support to our CFTSS/HCBS Program. This position will report to the Director of CFTSS/HCBS. This role is essentially the first point of contact for all referrals to the program. It will require great professionalism and advanced communication, organization and data entry skills.

Department: Children and Family Treatment Support Services (CFTSS) and Home and Community Based Service (HCBS) Program
Supervisor: Director of CFTSS/HCBS
Hours: 35 Hours; Monday through Friday; 9:00 a.m. to 5:00 p.m. with occasional flexible hours
Salary: $40,000 – $45,000
Responsibilities:
• Functions as the main point of contact for all referrals made to the CFTSS/HCBS Program and develops positive/helping relationships with referral sources to ensure a smooth referral process into the program. Troubleshoots with referral sources and supervisors to obtain the needed supporting documents for referrals.
• Reviews all referral paperwork and ensures that all necessary paperwork is received and that the client qualifies for the referred services before enrolling the client into the HCBS/CFTSS Referral Program. The client will then be put on the matching list and master spreadsheet.
• Reviews and Processes all referrals in the statewide IRAMS system before entering them into our NAC Electronic Health Record.
• Maintains all client referral documents and demographic information within the Electronic Health Record (EHR) and Microsoft One Drive. This includes the managing of several spreadsheets that assist in the tracking and monitoring of deadlines of Treatment Plans, Level of Care Eligibility and Authorizations.
• Maintain program client case records within the Electronic Health Record by inputting client demographics, collateral contacts, medical consenters, referral documents, name changes, etc. and by uploading referral documents, medical necessity forms and by ensuring that all documents are accurate.
• Will Check each clients Medicaid Eligibility at the beginning of each month.
• Manages all the training certificates and monitors training compliance for all the Clinical/Rehabilitation Service Providers (CRSP) and Administrative staff.
• Manages and distributes petty cash and MetroCards to CRSP staff as requested and appropriate.
• Manages all the application documents for potential CRSP staff and will communicate with the applicants if they are missing documents. Will also assist the Director/Assistant Director with the onboarding of new applicants. This includes, but is not limited to, calling references, uploading documents into the EHR, managing the training records for all new CRSPs and assigning the CRSPs cases in the EHR.
• Prepare reports, memos, letters and other documents using Microsoft word; excel spreadsheet,database, or presentation software. Responsible for sending out all mailers/letters to client families if needed.
• Attend program meetings and take minutes if requested.
• Serve as member of the program administrative team.
• Provide telephone reception coverage at front desk when necessary.
• Perform any other duties reasonably required to meet the responsibilities of the position.
Minimum Qualifications:
• Requires Bachelor’s degree and administrative support experience.
• Excellent communication skills and computer literacy required.
• Candidate must be highly organized, detail orientated, and able to handle multiple simultaneous projects and balance concurrent assignments.
• Computer skills must include MS Word, Excel, Access and Outlook.
• Communication skills must include written and oral proficiency, including effective telephone demeanor and the ability to work well with a variety of staff members.
