    Supervisor of CFTSS/HCBS

    NAC is currently seeking a Supervisor of CFTSS/HCBS. The CFTSS/HCBS Supervisor provides effective supervision to Clinical/Rehabilitative Service Provider (CRSP) staff, and has knowledge of the organization and the function of the department and its policies and procedures. In close collaboration with staff from multiple disciplines, (s) he supervises and supports the delivery of community based services. This position supports N.A.C.’s mission to provide the highest quality care by complying with regulatory guidelines and meeting agency standards.

    • Location: Midtown Manhattan (37 West 26th Street, 6th Floor, New York, NY 10010)
    • Department/Program: Children and Family Treatment and Support Services (CFTSS)/Home and Community Based Services (HCBS)

    Description of Responsibiliies:

    1. Provides monthly/more frequent one-on-one supervision to assigned service providers, approximately 22-30 providers. Supervising the clinical aspects of the work within the client’s home/community. This involves the development of crisis plans, monitoring the strength of relationships, understanding each client’s /family’s needs and addressing challenging situations within the family structure.  This also includes the development and execution of treatment plans.
    2. Continuously monitors service providers and the provision of in-home/telehealth services. This includes regularly confirming provider availability for work, referring cases to appropriate providers, communicating with referral sources and their supervisors, reading and approving encounter forms, and processing time sheets.
    3. Assists in the recruitment, screening, interviewing and hiring of service providers.
    4. Plans and implements in-service training opportunities that furthers the development of service provider skills and abilities. In addition, is responsible for the ongoing training of service providers on their case load.
    5. Participates in team meetings, case reviews, training and other program or agency-wide meetings as needed.
    6. Reviews billing and data entry reports for accuracy. Ensures that providers are offering services in accordance with family preferences and agency expectations.
    7. Conduct disciplinary meetings as needed; develop disciplinary plans for service providers and monitor the plans; terminating service providers if needed
    8. Covers department in the absence of Director and Assistance Director.
    9. Performs additional responsibilities in accordance with agency needs.

     

    Required Education and Experience:

    1. Master’s degree in Social Work, Mental Health Counseling or another LPHA (Licensed Practitioner of the Healing Arts)
    2. Minimum of one year of experience in programs providing services to medically complex children, which includes children with severe physical disabilities, emotional and behavioral challenges, and intellectual and developmental disabilities and their families.
    3. Knowledge of and experience in developing and managing programs a plus.
    4. Adept at collaborating with staff in a variety of different disciplines and programs.
    5. Ability and willingness to work a flexible schedule – particularly evenings and occasional Saturdays.

    Required Skills:

    1. LMSW, LMHC and/or another LPHA Preferred, Limited Permit Eligible
    2. Bilingual/Spanish communication skills preferred
    3. Excellent computer skills (Word processing, spreadsheet and web-based applications).
