What are Home Based Support Services?

NAC offers two NY State home and community health programs to support families on Medicaid caring for children with medical, developmental/intellectual and mental health needs: Home and Community Based Services (HCBS) and Children and Family Treatment and Support Services (CFTSS). 

These programs are designed to better meet children’s needs, expand access to clinical treatment services, and provide a greater array of approaches for rehabilitative interventions. It is vital that families on Medicaid have greater access to needed services and that they have the flexibility and choice to access what services are right for their child. 

HCBS and CFTSS services are voluntary and provide a family-driven, youth guided, culturally, and linguistically appropriate system of care which accounts for the strengths, preferences, and needs, as well as the desired outcome, of the individual. Trained staff provide an array of in-home and in-community services from therapeutic support to independent living skills and respite that are individualized to meet the health, developmental and/or behavioral health needs of each child/youth. The goal of the services are to assist the child/family to achieve a successful quality of life in the home, school, and community setting.


Children and Family Treatment and Support Services (CFTSS)

CFTSS is designed to meet the gap of services for Medicaid-eligible children and youth living in New York State. Families caring for eligible youth can access an array of clinical and/or rehabilitative home and community-based services that are delivered in the community where the child/youth lives, attends school, and/or engages in services. We provide individualized, one-on-one support that meets the unique needs of each child by highly trained Crisis Intervention Practitioners.  Services include: 

  • Home Based Therapy
  • Community Psychiatric Supports and Treatment 
  • Crisis Intervention
  • Psychosocial Rehabilitation
  • Youth Peer Support and Training
  • Family Peer Support Services

 

Who is eligible for services: 

  • Children and youth must be under 21 years old 
  • Children and youth must be eligible for Medicaid in New York State 
  • Children and youth applying for these services must have a Behavioral Health Diagnosis and/or are at risk for a behavioral health diagnosis

Home and Community Based Services (HCBS)

Children and youth enrolled in any Children’s Home Health Care program, including NAC’s Champions for Children’s Health [link Champions for Children’s Health] can be deemed eligible to receive services in the Home and Community Based Services (HCBS) program. 

Families who need extra support for their children with medical complexities, developmental/intellectual delays and behavioral health needs can access HCBS services, which are provided in the home or community where the child/youth resides. These services provide children with the opportunity to develop strong independent living, social, emotional and communication skills that will enhance their ability to have healthy relationships with other youth and adults in their lives. Services include: 

  • Community Habilitation
  • Caregiver/Family Supports and Services
  • Respite Services
  • Prevocational Services
  • Supported Employment
  • Community Self-Advocacy Training and Supports


Who is eligible for services:

  • Children and youth must be under 21 years old
  • Children and youth must be eligible for Medicaid in New York State
  • Children and youth must have an eligible medical, developmental or behavioral diagnosis
