New Alternatives for Children, Inc. (NAC) is an award-winning not-for-profit agency in Midtown Manhattan, with a satellite Bronx office, dedicated to serving children and families with medical complexities, chronic illnesses, and disabilities throughout the five boroughs. To assist in achieving this mission, NAC is currently seeking a Case Planner to join its Special Medical Prevention Program.
Our Prevention program delivers a range of clinical, case management, and supportive services to families who are child-welfare involved and caring for children diagnosed with chronic or complex medical conditions. The goal is for families to remain intact while ensuring caregivers receive the appropriate support to meet all of their children’s physical, developmental, and mental health needs.
The ideal candidate will be strength-based, trauma-informed, culturally responsive and committed to family preservation and resilience. This is a city-wide home and community-based program and the candidate must have the ability to work independently and flex their hours on weekdays to meet the needs of youth and caregivers. Our hybrid model supports work-from-home when staff are not in the field. This position offers a highly rewarding opportunity to build relationships with underserved families and an exciting opportunity to be part of an innovative multidisciplinary team comprised of pediatricians, nurses, psychiatrists, psychologists, clinicians, case associates, education and resource specialists, and more!
Description of Responsibilities:
Candidates receive credit toward NYS LCSW and LMHC licensing requirements. Free CEUs are available to licensed social workers (LMSW/LCSW) along with licensing prep courses for MSWs and MHCs who have completed probation.
